During the game, you take on the role of one of the TOOTHEADs - inhabitants of a trash-filled island where the remnants of human civilization fuel a fierce existence.

Eat or be eaten. Scour for better weapons, land precise hits, and torment your fellow players with sneaky tricks. And when everyone sharpens their teeth for you, play special cards or count on the mercy of the island. In short: do whatever it takes to survive. The game ends with the victory of the last player left at the table.

The expansion includes 4 new characters, 55 cards, and 6 dice, and also allows gameplay for 2 to 12 players. The heroes from the Heroes expansion have unique abilities, and Toothead's dice allocation is reversed: from a 12-sided die with 4 slices to a 6-sided die with 1 slice.
